Requirements for JAFAD College of Nursing Form

• A candidate who wants to apply for this admission form must have at least 5 credits in their WAEC or NECO it should not be more than two sittings, with chemistry, physics, mathematics and English and biology
• Applicant must have attained the age of 17years

• You can get the form at the campus of JAFAD College of Nursing Science, Oru-Ijebu North local government area Ogun State
• submit the completed admission form at the jafad college

NOTE: the completed form should be submitted with the following documents

1. photocopies of WAEC/GCE/NECO/NABTEB results
2. the copy of receipt/ teller for the application fee
3. two passports, with your full name written on it
4. three letters of recommendation/ attestation from individuals that can attest to your qualifications, characters and integrity

NOTE: Acceptable individuals that can provide references on your behalf are:

• head of the department of your current or previous employment
• your current or previous teacher (s)
• the principal of your previous or current school

The attestation letter should be signed and sealed by the above mentioned personnel, the letter will be rejected if it’s tampered with.

Oral Interview Schedule And Entrance Examination
